What an online semaglutide program should include
A legitimate online program is more than a shipment of medication. Look for:
- A medical evaluation by a provider licensed in your state, covering your health history, medications and BMI.
- A personalized dosing plan that increases gradually.
- Side-effect support — a way to reach the care team between refills.
- Licensed pharmacy fulfillment with clear labeling.
- Regular check-ins to adjust your plan.
How semaglutide works
Semaglutide is a GLP-1 receptor agonist. It mimics a natural hormone that helps regulate appetite and digestion — it slows how quickly your stomach empties and signals fullness to the brain, so many people feel satisfied with smaller portions. Read more about how semaglutide works.
A typical timeline
Weeks 1–4: getting started. Treatment begins at a low weekly dose so your body can adjust. Appetite changes often appear early; weight change is usually modest.
Months 2–4: dose increases. The dose is typically increased every four weeks, based on how you feel. This is when side effects like nausea are most likely — eating smaller meals, slowing down and limiting greasy food helps.
Months 4–12: steady progress. Once at a maintenance dose, weight loss tends to continue steadily. In the STEP 1 trial, adults taking semaglutide 2.4 mg weekly lost about 15% of body weight on average at 68 weeks, versus about 2% on placebo. Everyone's results are different. See the week-by-week guide.
Side effects and safety
The most common side effects are gastrointestinal: nausea, diarrhea, constipation, vomiting and stomach pain. Less common but serious risks include pancreatitis and gallbladder problems. Semaglutide carries a warning about thyroid C-cell tumors seen in animal studies and is not used in people with a personal or family history of medullary thyroid cancer or MEN 2. Is semaglutide safe?
