Key Takeaways

  • 1FDA labeling for weight management covers adults with BMI ≥30, or BMI ≥27 with at least one weight-related condition.
  • 2Qualifying conditions include high blood pressure, type 2 diabetes, high cholesterol, sleep apnea and heart disease.
  • 3GLP-1s are generally not used during pregnancy or with a personal or family history of medullary thyroid cancer or MEN 2.
  • 4A licensed provider makes the final decision after reviewing your full health history.

The BMI thresholds

FDA labeling for GLP-1 medications approved for chronic weight management (Wegovy and Zepbound) covers adults with:

  • BMI of 30 or higher, or
  • BMI of 27 or higher plus at least one weight-related condition.

BMI is your weight in kilograms divided by your height in meters squared. For example, someone who is 5'6" reaches a BMI of 27 at about 168 lb and a BMI of 30 at about 186 lb.

Weight-related conditions that may qualify

  • High blood pressure
  • Type 2 diabetes or prediabetes
  • High cholesterol or triglycerides
  • Obstructive sleep apnea
  • Cardiovascular disease

Reasons a provider may say no

GLP-1 medications are generally not prescribed if you:

  • are pregnant, planning pregnancy or breastfeeding;
  • have a personal or family history of medullary thyroid cancer or MEN 2;
  • have had pancreatitis, or have certain severe digestive conditions;
  • have a history of serious allergic reaction to a GLP-1.

Your provider will also review other medications — especially insulin or sulfonylureas, which can raise the risk of low blood sugar — and conditions such as kidney disease or a history of eating disorders.

How the decision is made

Eligibility isn't just a number. A licensed provider looks at your BMI, health history, medications, goals and safety factors together. If medication isn't right for you, a good program will tell you why and suggest other options.
