The BMI thresholds
FDA labeling for GLP-1 medications approved for chronic weight management (Wegovy and Zepbound) covers adults with:
- BMI of 30 or higher, or
- BMI of 27 or higher plus at least one weight-related condition.
BMI is your weight in kilograms divided by your height in meters squared. For example, someone who is 5'6" reaches a BMI of 27 at about 168 lb and a BMI of 30 at about 186 lb.
Weight-related conditions that may qualify
- High blood pressure
- Type 2 diabetes or prediabetes
- High cholesterol or triglycerides
- Obstructive sleep apnea
- Cardiovascular disease
Reasons a provider may say no
GLP-1 medications are generally not prescribed if you:
- are pregnant, planning pregnancy or breastfeeding;
- have a personal or family history of medullary thyroid cancer or MEN 2;
- have had pancreatitis, or have certain severe digestive conditions;
- have a history of serious allergic reaction to a GLP-1.
Your provider will also review other medications — especially insulin or sulfonylureas, which can raise the risk of low blood sugar — and conditions such as kidney disease or a history of eating disorders.
How the decision is made
Eligibility isn't just a number. A licensed provider looks at your BMI, health history, medications, goals and safety factors together. If medication isn't right for you, a good program will tell you why and suggest other options.
